Nomination forms can be sent via online submission. Alternatively, the form is available as a printable version which can be sent via postal mail. Online submissions are preferred.
Printed forms should be sent as postal submission or via email. Completed forms can be emailed to admin@aahof.com.au with the subject as Completed Nominee Form.
If sending as a postal submission;
Print the above form and fill out.
Please send copies of documents only.
The envelope must be marked with the nomination name, return address, and sent to the following address:
